What it assesses
The test examines the carotid arteries (common, internal and external) and the vertebral arteries along their course in the neck. It measures the thickness of the artery wall, identifies atherosclerotic plaques and describes their features, and estimates the degree of any narrowing (stenosis) by measuring flow velocity. It also assesses the direction of flow in the vertebral arteries.
When it is indicated
- after a stroke or TIA;
- in the presence of cardiovascular risk factors such as high blood pressure, diabetes, smoking or high cholesterol;
- to monitor known plaques or stenosis over time and after carotid procedures (endarterectomy or stenting);
- in some cases of vertigo or transient visual disturbances.
How it is performed
The test takes about 20 minutes, lying down with the neck slightly extended: an ultrasound probe with gel is moved along both sides of the neck. No preparation or fasting is needed; it helps to avoid necklaces and high-necked clothing.
The report
The report describes the condition of the artery walls, any plaques and the percentage of stenosis, with recommendations for follow-up. When needed, the test is complemented by study of the intracranial arteries with transcranial Doppler or with MR or CT angiography.
